This draft PR implements the transition to HTML form for the FAST checklist.

Note that this is one of the 2 versions that is being worked on.

• Current version - The results of the form will be copied to the clipboard in a table format, matching the appearance of the HTML form, which could then be pasted into a Github issue in your spec's repository.
• Second version - The results of the form are copied to the clipboard in a list format, which can then be pasted into a GitHub issue in your spec's repository. Here's the DRAFT PR for the gh-list version.

What remains the same across both versions is the HTML form itself. The difference lies only in how the results are copied to the clipboard.
